新型インフルエンザの二次感染(secondary respiratory infections、secondary transmission、double whammy-二重苦- )とは、渡航歴のない人の感染ということだが。
これまでに、二次感染者と見られるのは、
イギリス1人
ドイツ1人
スペイン1人
カナダ1人
韓国2人ー疑い
ということだ。
二次感染の場所は、

�３惺�(ただし、アメリカでの感染も二次感染とみなした場合)
などとなっている。
ニーマン博士は、この2ー3日以内にフェーズ6との見通しをしていたが、この格上げは、これらの二次感染の拡大しだいということになるのだろう。
そこで、いつフェーズ6への移行があるのか、だが、ここ数日という見方が濃厚である。
なぜなら、WHOのフェーズは、感染の地理的な拡大を示す指標であって、感染ウイルスの劇症性を示すものではないからだ。
今日のニューヨークタイムズでは、WHOの警告レベル担当チーム(緊急対策部長)のライアン博士(Dr. Michael J. Ryan)の電話インタビューの言として、つぎのような記事を載せている。
概訳
ライアン博士は、「現時点で、私としては、バンデミックは差し迫っている、と、提言している。
なぜなら、われわれは、他国への伝染を見ているからだ。」
といっている。
そして、「われわれは、フェーズ6の段階に到達するであろうと、予測せざるを得ないと見ている。そうならないことを望むものではあるが—」
ともいう。
WHOにおいての最高の警告レベルであるフェーズ6段階は、パンデミックのレベルである。
しかし、ライアン博士は、次のことを強調している。
すなわち、世界的なパンデミックは、疾病の地理的な広がりを示すもので、その疾病の重大性を示すものではない。
症状的に軽い疾病のパンデミックもありうる。
現在の警告レベルであるフェーズ5は、次のことを示している。
すなわち、疾病が、家庭内にとどまらず、コミュニティレベルに広がっていることとか、WHO加盟6管轄地域のなかの二つの国-今回の場合は、メキシコと北米-での旅行者の本国帰国者の間に広まっていること、を示している。
これをフェーズ6に格上げするためには、コミュニティの広がり(二次感染のことか?)が、少なくとも、他の地域の一国に発生する必要がある。
ライアン博士は、こうも言う。
「私の思うに、今あるこの現象は、ここ数十年にわたって、われわれが気付いていたことだったのです。」
そして、博士のいうに、この間の公衆衛生への脅威は、動物を起源とするものがほとんどであり、ウイルスがひとつの種から他の種へとジャンプすることで、大破壊を生んできたのだという。
「われわれは、過去にも、豚からヒトへの疾病の拡大を見てきました。
しかし、今回の場合は、豚がヒトへ疾病を伝染したという確たる証拠がないまま、ヒトからヒトへの伝染にいたっているのです。
ここで、われわれは、動物ーヒト間のインターフェイスについて、注意深く見ていかなくてはならないでしょう。」
といっている。
“At the present time, I would still propose that a pandemic is imminent because we are seeing transmission to other countries,”
“We have to expect that phase 6 will be reached. We have to hope that it is not.”
Phase 6, the highest level in the organization’s health alert system, is a pandemic. But Dr. Ryan emphasized that the word pandemic describes the geographic spread of a disease, not its severity. There can be a pandemic of a mild disease. The current level, phase 5, means that the disease is spreading in communities — not just within households or in returning travelers — in two countries in one of the World Health Organization’s six regions, in this case the United States and Mexico. To move up to phase 6, community spread would have to occur in at least one other country in another region. ”
“I think this is a phenomenon we’ve been observing over the last few decades,” Dr. Ryan said. He noted that some of the major public health threats were of animal origin, including viruses that can wreak havoc when they jump from one species to another.
“We have seen in the past that disease can spread from pigs to humans,” Dr. Ryan of the W.H.O. said. “It usually dead ends with one or two cases.”
But in this case, he said, the disease is now spreading from person to person, with no evidence that pigs were transmitting it to people.
Still, he said, “the animal-human interface needs to be watched carefully.”
